To send your testimony,
please follows these instructions:
Please complete and sign
the authorization form,
and enclose it with your testimony. If
the authorization form is
not signed, we cannot publish the testimony.
The testimony should be typed
by word processor or typewriter. If you decide to handwrite
your testimony write legibly and please print. If we cannot
read your testimony, we cannot publish it.
If you are under
21, a parent or guardian needs to sign the authorization form.
As soon as
the book is finished, we will notify you by mail .
